RESUMO

PURPOSE: to assess prevalence of dyslipidemia in patients with ischemic heart disease (IHD) older than 75 years as well as to evaluate possible associations between serum lipids and various cardiovascular and other diseases in these patients. METHODS: We enrolled in this cross sectional study 555 hospitalized IHD patients aged 75-98 years (mean age 86.8 years, 74.5 % women). Levels of lipids (total cholesterol [TC], triglycerides [TG], low and high-density lipoprotein cholesterol [LDLC, HDLC]) were measured by enzyme method on the biochemistry analyzer Konelab 60i. RESULTS: Elevated TC; hypertriglyceridemia and elevated LDLC were observed in 13.3, 10.4 and 26.3 % of patients, respectively. In the majority of patients severity of dyslipidemia was mild. With increasing age serum levels of TC and LDLC decreased. Negative correlation between TC level and patient's age was significant (r= -0.13; p=0.001). We examined 39 women with normal endometrium and 139 women with glandular-cystic hyperplasia of the endometrium (without atypia). According to clinical manifestations of hyperplasia, the patients were divided into 3 groups: 74 (53%) had reestablishing menstrual function after total curettage (group 1); 42 patients (30%) with glandular-cystic hyperplasia after curettage and hormonal therapy with progesterone and synthetic progestins (duration 3 to 6 months) had no repeated pathology of the endometrium (group 2); endometrial hyperplasia recurred 2 and 3 times as showed biopsies during 2-5 years of observation in 23 (17%) women (group 3). Immunohistochemical tests of normal endometrium revealed correlations between stages of menstrual cycle and steroid hormone receptors in nuclei of glandular epithelium and stromal cells. Maximum sensitivity of glandular epithelium to estrogen and pronounced expression of estrogenic receptors were observed at middle and late stages of proliferation. High sensitivity of glandular epithelium to progesterone was registered at middle and late stages of proliferation and early stage of secretion. Two types of hormone receptor expression were observed. Type 1 typical for the endometrium of middle and late stage of proliferation was characterised by a high content of receptors to E2 and P in glandular epithelium and stromal cells. Type 2 was observed in patients with recurring glandular hyperplasia and was characterised by a mosaic picture up to complete absence of receptor expression in nuclei of some glands and stromal cells. The detected zones free of receptors to estrogens and progesterone evidence for local disturbance of a regulating role of signal pathways of sexual steroids and can serve a substrate for formation of tissue autonomy.

RESUMO

No appreciable disorders of cellular immunity were detected in patients with glandular cystic endometrial hyperplasia. Atypical endometrial hyperplasia was associated with quantitative changes in T lymphocytes and their subpopulations, decreased level of lymphocytes carrying activation antigens, and increased count of natural killers. These changes can be characterized as immunocompensation.

RESUMO

53 histological slides obtained from 4 patients aged 35-68 years with diagnosis of uterine carcinoma without invasion (16 patients) and tumor invasion into uterine muscular membrane (24 patients) were retrospectively analyzed. On the basis of morphometry and ploidometry of 2989 nuclei on the image analyzer Imager-CG (Russia) with a computer program Avtan-San, a complex of diagnostic criteria characterizing the grade of malignancy of uterine tumors was obtained. Differential diagnostic ploidometric characteristics of 4 degrees of tumor progression are described. With the process intensification the amount of genetic material in tumor cell nuclei increase 1.7-fold, proliferative activity 2.2-fold, number of polyploid cells--1.5-fold. These data specify the degree of differentiation of uterine body adenocarcinoma and help to plan treatment policy for patients with uterine carcinoma.

RESUMO

A solid-phase enzyme immunoassay of human lactoferrin was developed, which was used in diagnosis of some septic diseases. The test system developed enabled lactoferrin to be measured in the blood serum and plasma of healthy volunteers: 955.8 +/- 74.4 and 405.5 +/- 27.7 mg/e, respectively. This assay may be used also in measurement of blood serum lactoferrin levels in various pathological states and particularly applied for detection of postnatal pyoseptic failures before their clinical manifestations.

RESUMO

A total of 256 pregnant women were examined in pregnancy terms of 19-40 weeks. Besides routine fetometric parameters, the transverse diameter of the cerebellum was measured in the fetuses. This parameter was in good correlation with the gestational age. In 42 cases with small-for-date fetuses the transverse diameter of the cerebellum did not much differ from that in the reference group. Therefore measurement of the transverse diameter of the cerebellum is a truly good method for the assessment of the gestational age, for it is virtually independent of all the pathologic processes in the mother or fetus.

Comprehensive studies of the fetoplacental system in pregnant women with gestoses of varying severity (n = 260) and in those with normal pregnancy (n = 200), carried out in pregnancy terms of 29 to 41 weeks, have revealed a lowering of the levels of trophoblastic beta-globulin and pregnancy-associated alpha 2-glycoprotein, elevated concentration of pregnancy-associated protein A, a higher incidence of placental types II and III (as evidenced by ultrasonic examination), reduced rate of growth of the fetal biometric parameters and a reduction of fetal systolic output, augmenting with the gestosis severity. A comprehensive assessment of the fetoplacental system permits obtain a more complete picture of the gestosis severity.

RESUMO

The activities of the blood kallikrein-kinin system components (plasma kallikrein, alpha 1-antitrypsin, alpha 2-macroglobulin), and gonadotropin levels in various phases of the menstrual cycle were measured in 35 women ob various age groups. The same parameters were investigated in 28 women with dysfunctional uterine bleedings of the climacteric period. Phasic changes in the blood kallikrein-kinin system components, closely related to the hormonal status, were revealed over the course of the onto-genesis. Climacteric dysfunctional uterine bleedings were found associated with an elevated activity of the plasma kallikrein, parallelled by a relative insufficiency of the blood plasma inhibitor potential. A relationship between the activities of the blood kallikrein-kinin system components and endometrial function has been revealed. These findings necessitate search for approaches to pharmacologic regulation of the blood kallikrein-kinin system activity and to pathogenetic therapy of the climacteric dysfunctional uterine bleedings.

RESUMO

A regimen of fluid and transfusion therapy has been developed on the basis of clinical and laboratory evaluation of postresuscitation course and outcomes of parturients with massive blood loss and a history of gestosis. An early introduction and systematic use of proteolytic inhibitors were beneficial. Surgical hemostasis warrants a radical approach and requires ligation of internal abdominal arteries and uterus extirpation.

The paper presents a discussion on therapeutic results obtained with plasmapheresis treatment in 16 patients with stage II essential hypertension. Thirty-three comparable patients entered a control group of conventional treatment. The test and control patients with stage II hypertension were examined for time-course changes in T- and B-lymphocytes counts, the activity of the energetic enzymes (alpha-glycerophosphate dehydrogenase, succinic dehydrogenase, lactate dehydrogenase), serum immunoglobulins and circulating immune complex levels. It was established that the standard antihypertensive treatment failed to restore normal parameters of immunity and to favor positive alterations in the activity of the enzymes in the blood lymphocytes, whereas therapeutic plasmapheresis was found to stimulate immunity, especially cellular one. This occurred in line with a rise in the levels of lymphocytic dehydrogenases.

RESUMO

A survey of cesarean sections performed between 1984 and 1987 has demonstrated a changing pattern of indications for abdominal delivery. An expanded range of indications for cesarean section in cases of breech presentation and maternal gestosis has been shown to reduce significantly perinatal losses. It is demonstrated that indications for vaginal delivery in patients with a uterine scar should be expanded through further search for new methods for the assessment of the scar as well as the improvement of the available ones. Intraoperative administration of antibiotics is a method of choice in preventive antibiotic treatment.
